Referral Process

The Intake Department at LifeStance Health is available to assist you throughout the referral process by scheduling patient appointments and answering any questions you or your patient may have.

Referrals are placed by one of our trained Intake Specialists, either through a systematic matching process or as a direct referral of your choice. Intake Specialists also can guide you in determining appropriate services or programs for your patients as they begin or continue their behavioral health care journey. 

Each LifeStance Health clinician follows a problem-solving approach to therapy and treatment, which includes comprehensive evaluation, collaborative development of goals, and psychiatric assessment and medication when appropriate.

To schedule an appointment, LifeStance Health will need the following information:

  • Patient Name
  • Address
  • Phone number
  • Date of birth
  • Insurance information
  • Nature of behavioral health issue or relevant diagnoses

Patient Outcomes Report
